If you’re looking for your personalized health provider screening form, a link to schedule an onsite screening managed by Aduro, or information about other health screening options available to your program, you’ll find them on your program's WellMetrics page.
Please note that options will differ by program, and that onsite health screenings might be available through Aduro, or through a third-party vendor contracted separately by your employer.
See below for instructions on finding the WellMetrics page on the program web site and on the Aduro Mobile App.
Web Instructions
1. Log in to your program website and click the Me link in the upper right. In the pop-up that opens, click the WellMetrics tile.
2. On the WellMetrics page, scroll down to find the links to your available health screening options.
If your program offers onsite health screenings managed by Aduro, the Schedule a Screening option will be displayed. Click the link and follow the prompts to schedule an appointment.
For all other options, including downloading the health provider screening form, click the Select Alternative Options link.
3. The Alternative Options page includes a link to download your personalized health provider screening form, as well as a link to upload the completed form to us.
Aduro Mobile App Instructions
1. Log in to the Aduro Mobile App and click the Me link in the lower right. On the next screen, click the WellMetrics tile.
2. On the WellMetrics page, scroll down to find the links to your available health screening options.
If your program offers onsite health screenings managed by Aduro, the Schedule a Screening option will be displayed. Click the link and follow the prompts to schedule an appointment.
For all other options, including downloading the health provider screening form, click the Select Alternative Options link.
3. Scroll down the Alternative Options page to find a link to download your personalized health provider screening form, as well as a link to upload the completed form to us.
