See the instructions below to schedule an onsite health screening appointment through eHealthScreenings (if available). Options will differ by program. Check your program's health screening tile or WellMetrics page for details.
- eHealthScreenings is one of our onsite health screening partners. Options will differ by program. Onsite health screenings are conducted at locations selected by your well-being program provider/employer, and not at an eHealthScreenings Patient Service Center.
Schedule an onsite appointment via your program website
1. Log in to your well-being program website and click the health screening Activity tile (tile names will vary). On the activity description page, scroll down and Join/Enroll in the activity, if you haven't already, then click the Health Screening Options button.
2. On the Health Screening Options page, scroll down and click the Onsite Screening button. In the pop-up that opens, click the Schedule Screening button to transfer to the eHealthScreenings website.
3. On the eHealthScreenings website, you will log in automatically via your well-being program account. If any required items are missing from your profile, such as your phone number, you will be prompted to provide them. Once this information is filled in, click the Continue button.
4. On the next screen, you can choose whether to opt-in for text messaging. If you do so, you'll receive a reminder 14 hours prior to your appointment. This reminder would be a good way to ensure that you fast ahead of time. Once you've made your selection, click the Submit button.
5. On the next screen, click the Please select a state drop-down menu and select the state that is appropriate for you. You will then see the available locations and dates for that state. Scroll down to find an event, and then click the Please make a selection drop-down menu for that event. Scroll through the available time slots and select one that is convenient for you. Click the Register button to confirm your choice.
6. On the next screen, follow the prompts to select your comfort level, and then confirm your answer.
Note that the comfort level question will only be presented for screening events featuring blood draw, rather than finger stick.
7. On the next screen, read through and sign the screening consent form. Note that you must scroll through the entire consent before you can click the I Agree checkbox. Then fill in your name and click the Continue button to proceed.
8. The confirmation screen will display your appointment details and give you the option to add it to your calendar. You will receive an email reminder 24 hours prior to your scheduled appointment time, whether or not you opted for text messaging.
9. To check your appointment details, or to reschedule or cancel your appointment, repeat steps 1-2 above to return to the eHealthScreenings website.
