The information in this article applies to Castlight/apree health members whose programs include Aduro WellMetrics services.
See the instructions below to schedule a Quest Patient Service Center appointment, if offered through your program. Options will differ by program.
- Note that this a standalone option. If you plan to submit the Health Provider Screening Form (HPSF), and your health provider sends you to a Quest location for a blood draw, do NOT use the process outlined below. For instructions on using the HPSF, click here.
Schedule a Quest Patient Service Center (PSC) appointment
1. Log in to your program at https://us.castlighthealth.com/ or use the Castlight mobile app.
2. Click on the Benefits menu, and then click Browse All. Scroll through your available benefits and click the Aduro Wellmetrics program.
3. Locate the Health Screening activity (titles may vary), and click the Go button.
4. Create an Aduro profile if you have not done so already. If you created an Aduro profile previously, this step will be skipped automatically.
5. Locate the Visit your local Quest facility option, and click the Schedule Screening button to transfer to the Quest Diagnostics website. Follow the prompts to schedule a screening.
6. If this is your first visit to the Quest website through your well-being program, you will be asked to read and accept Quest's Terms and Conditions, and create a profile.
7. On the next screen, locate the Wellness Screening section. Under the At a Patient Service Center option, click the Make an Appointment button.
8. Use the map to find a convenient location. Click the location of your choice in the list to the right of the map, then click the Continue button.
9. Next, click the Choose a date option and then the Choose a time option, and make your selections for each. Next, click the Continue button and follow the prompts.
10. Once you've confirmed your appointment, you'll see a screen with the appointment details and a link to download and print your order requisition. This is a form that includes your appointment details, and lets Quest know to bill the screening, and forward the results, to your well-being program.
11. If you need to see your appointment details or reschedule/cancel your appointment, follow the directions above to return to the Quest scheduler. Click the appropriate link to reschedule or cancel.
12. Be sure to take your printed order requisition form to your appointment. If you need assistance with your appointment, please click the Submit a Request link at the top of this page.
