Order a home test kit via the Aduro Mobile App
Order a home test kit via your program website
1. Log in to your well-being program website and click the health screening Activity tile (tile names will vary). On the activity description page, scroll down and click the Health Screening Options button.
2. On the Health Screening Options page, scroll down and click the Home Health Kit option.
3. In the pop-up that opens, click the Order Home Health Kit button. Note that once you've ordered a kit, a confirmation message with the order date will be displayed here.
4. Fill out the order form and click the Consent for Testing checkbox near the bottom. Note that you must provide a valid ten-digit phone number (including area code) and a valid email address. Be sure that the personal information on the home test kit order form and/or included with the returned sample matches what is on your well-being program account. Click the Order button to submit your order. Allow 3 - 5 business days for delivery.
Order a home test kit via the Aduro Mobile App
1. Log in to the Aduro Mobile App using the same credentials you use for your well-being program website. Tap the Home icon on the bottom menu bar.
2. Locate and tap the health screening Activity tile (tile names will vary). On the activity description page, scroll down and tap the Health Screening Options button.
3. On the Health Screening Options page, scroll down and tap the Home Health Kit button. In the pop-up that opens, tap the Order Home Health Kit button. Note that once you've ordered a kit, a confirmation message with the order date will be displayed here.

Click the chat icon (speech bubble) in the lower right corner of the page to connect with a live agent, or click the Submit a Request link at the top of the page, and we'll respond as soon as possible. When submitting a help request, please include the following information as applicable:
- Your name
- Email address associated with your well-being program account
- Your phone number and time zone (we'll need to speak to you personally for account, personal, or protected health information issues)
- Your well-being program provider/employer
- Device you’re using and the operating system
- Web browser you’re using and the version number, OR Aduro Mobile App version number (when logged in, tap the menu icon in the upper right)
- Step-by-step description of the issue, including the names of any Activities, Paths, etc. involved
- Screenshots that illustrate the issue, if possible
