Check your program's WellMetrics page to find details on your health screening options. See the instructions below to use the Health Provider Screening form (HPSF) option (if available). Options will differ by program.
- Be mindful of your program's health screening deadline to receive proper credit.
- Forms must be filled out completely and signed by both the member and their health care provider to be processed.
- If you received an HPSF from an Aduro agent with no preprinted personal information, the upload website in the Completion Directions will not work. The blank forms are generic and not specific to your program. Please refer to the agent's email for the submission details.
- Note that a health screening with or ordered by your provider is subject to normal patient fees, co-pays, etc. If you plan to submit the health provider screening form, and your provider sends you to a LabCorp location for a blood draw, do NOT use the LabCorp voucher. The LabCorp voucher is only used as a standalone option.
1. Schedule a health screening as directed by your health care provider. Your screening might be with your provider or with a third-party testing facility.
2. Download the HPSF from your program website or the Aduro Mobile App.

- Log in to your program website, click the Me link in the upper right, then click the WellMetrics tile.
- On the WellMetrics page, scroll down and click the button that says Screening Options, or something similar. The page that opens includes a download link, as well as a link to upload the form once it's filled out.

- Log in to the app and tap the Me (person) icon on the lower menu bar, then tap the WellMetrics tile.
- On the WellMetrics page, scroll down and tap the button that says Screening Options, or something similar. The page that opens includes a download link, as well as a link to upload the form once it's filled out.

3. Print the form, and then complete and sign the EMPLOYEE portion of the form.
4. Review the Completion Directions at the top of the form. Example:
5. Bring the form to your provider at the time of your screening or once your results are available to have them complete the lower portions of the form.
6. Once the provider has completed the form, we recommend that you retrieve the form for your records and to submit it yourself.
7. Follow the submission instructions in the Completion Directions. Allow up to three business days for processing.
