Check your program's WellMetrics page to find details on your health screening options. See the instructions below to use the LabCorp voucher option (if available). Options will differ by program.
- Note that the LabCorp voucher is only used as a standalone option. If you plan to submit the health provider screening form, and your provider sends you to a LabCorp location for a blood draw, do NOT use the LabCorp voucher. In this case, the screening is subject to normal patient fees, co-pays, etc.

1. Log in to your program website and click Me in the upper right. Then click the WellMetrics tile.
2. On the WellMetrics page, scroll down and click Screening Options.
3. Instructions for visiting a LabCorp facility will be on the right (if available in your program).
4. Click on the Download button to download a printable copy of the LabCorp voucher form. You MUST print this form and take it with you to your LabCorp appointment!
5. You MUST make an appointment at a LabCorp facility! Click on the https://www.labcorp.com/wps/portal/findalab link to locate a LabCorp Patient Service Center (PSC). You will be directed to the website below - enter your address or zip code and be sure to select Employee Wellness with body measurement. Click Search to find the nearest LabCorp that offers Employee Wellness services.
6. After you locate your preferred LabCorp facility, click Make Appointment.
7. Be sure the service still says Employee Wellness with body measurement and click Yes for fasting (you must be fasting for 12 hours for the Lipid/Cholesterol and Glucose results to be accurate). Then click Choose Date and Time.
8. Choose your preferred appointment date and time, then click Select.
9. Click the I'm not a robot box and then click Next. You will then enter your personal information and confirm your appointment details.
10. Be sure to take your printed LabCorp voucher form to your appointment. LabCorp will send your results to Aduro via their LabCorp Link. Your results will be available in your Wellmetrics tile in 3-7 business days.

2. Scroll down to Wellmetrics, and click View Biometric Scores.
3. Scroll to the bottom on the page and click Screening Options.
4. Scroll down to the "Visit your local LabCorp facility" section (if available in your program).
5. Click on the Download button to download a printable copy of the LabCorp voucher form. You MUST print this form and take it with you to your LabCorp appointment!
