See the instructions below to schedule a Quest Patient Service Center appointment (if available). Options will differ by program. Check your program's health screening tile or WellMetrics page for details.
- Note that this a standalone option. If you plan to submit the Health Provider Screening Form (HPSF), and your health provider sends you to a Quest location for a blood draw, do NOT use the process outlined below, but visit the Quest Diagnostics website directly. Schedule a Quest Patient Service Center (PSC) appointment via your program website
1. Log in to your well-being program website and click the health screening Activity tile (tile names will vary). On the activity description page, scroll down and click the click the Health Screening Options button.
2. On the Health Screening Options page, scroll down and click the Patient Service Center option.
3. In the pop-up that opens, click the Schedule Screening button to transfer to the Quest Diagnostics website. Follow the prompts to schedule a screening.
4. If this is your first visit to the Quest website through your well-being program, you will be asked to read and accept Quest's Terms and Conditions, and create a profile.
5. On the next screen, locate the Wellness Screening section. Under the At a Patient Service Center option, click the Make an Appointment button.
6. Use the map to find a convenient location. Click the location of your choice in the list to the right of the map, then click the Continue button.
7. Next, click the Choose a date option and then the Choose a time option, and make your selections for each. Next, click the Continue button and follow the prompts.
8. Once you've confirmed your appointment, you'll see a screen with the appointment details and a link to download and print your order requisition. This is a form that includes your appointment details, and lets Quest know to bill the screening, and forward the results, to your well-being program.
9. If you need to see your appointment details or reschedule/cancel your appointment, follow the directions above to return to the Quest scheduler. Click the appropriate link to reschedule or cancel.
10. Be sure to take your printed order requisition form to your appointment. If you need assistance with your appointment, please click the Submit a Request link at the top of this page.

1. Log in to the Aduro Mobile App using the same credentials you use for your well-being program website. Tap the Home icon on the bottom menu bar.
2. Locate and tap the health screening Activity tile (tile names will vary). On the activity description page, scroll down and tap the Health Screening Options button.
3. On the Health Screening Options page, scroll down and tap the Patient Service Center button. In the pop-up that opens, tap the Schedule Screening button to transfer to the Quest Diagnostics website. Follow the prompts to schedule a screening.

When submitting a help request, please include the following information as applicable:
- Your name
- Email address associated with your well-being program account
- Your phone number and time zone (we'll need to speak to you personally for account, personal, or protected health information issues)
- Your well-being program provider/employer
- Device you’re using and the operating system
- Web browser you’re using and the version number, OR Aduro Mobile App version number (when logged in, tap the menu icon in the upper right)
- Step-by-step description of the issue, including the names of any Activities, Paths, etc. involved
- Screenshots that illustrate the issue, if possible
