Connect Care for Diabetes provides valuable tools and impactful support for those with Type 2 Diabetes, and is available to select well-being programs and members.

Program participants may be eligible to receive a free continuous glucose monitor (CGM) device for the duration of the program.

• Participants who receive a CGM can then share their data with the Aduro platform, allowing this data to display automatically in the Connect Care for Diabetes Blood Glucose tab.
• Participants without CGMs can manually track their blood glucose finger stick readings in the Connect Care for Diabetes Blood Glucose tab.

To find out if you are eligible to receive a CGM, first enroll in Connect Care for Diabetes: Connect Care for Diabetes - Getting started.

Once you enroll, you will receive a survey via email. Please complete the survey to determine your CGM eligibility.
