Connect Care for Diabetes is Aduro's program for supporting members who have Type 2 Diabetes, and is available to select programs and members.
Program participants may be eligible to receive a continuous glucose monitor (CGM) device for the duration of the program. Once participants complete the onboarding process, they receive a survey via email to determine their CGM eligibility.
- Participants who receive a CGM can then share their data with the Aduro platform, allowing this data to display automatically in the Connect Care for Diabetes Blood Glucose tab.
- Participants without CGMs can manually track their blood glucose finger stick readings in the Connect Care for Diabetes Blood Glucose tab.
- See Connect Care for Diabetes - Blood glucose tab navigation and Connect Care for Diabetes - Submit a blood glucose entry.
When using a Dexcom CGM, note that finger stick test readings and CGM readings might not match exactly.
However, if the readings are within a certain percentage of each other, they are considered close. Please click here to see Dexcom's discussion of this topic.
If you have questions on getting started with the CGM, data sharing, or a sensor refill, contact us at ConnectCareCGM@adurolife.com.
You can also discuss any health concerns with your primary health care provider, or if the issue is regarding CGM functionality and accuracy, contact Dexcom support on their website: https://www.dexcom.com/.
